You will be responsible for undertaking assessments of need, care management and other duties relating to the individuals, families and carers allocated to them in accordance with the Care Act 2014 and other statutory instruments, policy and procedures relating to the social care of adults. You will ensure that service users and carers are kept safe through risk management and Section 42 processes. You will also be able liaise and co-ordinate services provided to those people through individualised support planning and the use of Personal Budgets.
