We currently have five newly created posts for you to provide practical and administrative support to the Assisted Discharge Service Co-ordinator, Assistant Co-ordinator and a team of volunteers. As the Team Support Worker you will deliver the person-centred emotional and practical assistance within the timeframe and criteria stipulated by the Assisted Discharge service and in accordance with Red Cross policies.

The positions we have available are: 1 @ 21 hours per week, Sunday - Thursday 12 - 8pm 2 @ 16 hours per week, Wednesday- Saturday 6 -10pm 2 @ 16 hours per week, Sunday -Tuesday 6 - 10pm   The successful candidates will assist with the development of community risk assessments and ensure they are adhered to by staff at all times. The post-holders will assist with the transport of medically fit patients from the hospital to their homes and ensure they are settled in and comfortable. This service will run for 7 days a week over the pilot period.   You will also maintain and update administrative and communication systems in support of high quality service delivery, assist with the assessment of clients in the hospital setting. The main aim of this service is to prevent and reduce hospital admissions.   You will be responsible for liaising with various departments promoting and raising the profile of the service. During periods of absence of the Service Co-ordinator and Assistant co-ordinator, the post holder will act as a focal point to the hospital referral teams and any other stakeholders. This is a role which can be unpredictable and fast paced and requires an excellent organiser and communicator - someone who is a self-starter wanting to make a real difference to people's lives. We are a volunteer-led humanitarian organisation that helps people in crisis, whoeve rand wherever they are. We enable vulnerable people at home and overseas to prepare for and respond to emergencies in their own communities.
