This is a new post, which aims to provide practical and administrative support to the Assisted Discharge Service Coordinator, Assistant Coordinator and a team of volunteers. As the Team Support Worker, you will ensure that the volunteers deliver the person-centred emotional and practical assistance within the timeframe and criteria stipulated by the Assisted Discharge service and in accordance with Red Cross policies. This service will run for 7 days a week over the pilot period.

The post holder will maintain and update administrative and communication systems in support of high quality service delivery, assist with the assessment of clients in the hospital setting. The main aim of this service is to prevent and reduce hospital admissions.

You will be responsible for supervising a team of volunteers based at the hospital and liaising with various departments promoting and raising the profile of the service.
