This role is an exciting opportunity to help implement and develop the set up and delivery of the new A&E Assisted Discharge service in Brighton and Hove, where vulnerable patients are transported and resettled back home. The hours of this post are 10am - 6pm Sunday - Tuesday and will be shared with other staff to cover a seven day period.

The team will support the transportation and resettlement of vulnerable patients from hospital to their home. The main aim of this service is to prevent and reduce hospital admissions. 

You will be responsible for supporting the service coordinator and be supervising a team of support staff based at the hospital and liaising with various departments promoting and raising the profile of the service.  You will support case coordination, documentation, risk assessment, monitoring and evaluation of the service. This is a role which can be unpredictable and fast paced and requires an excellent organiser and communicator - someone who is a self-starter wanting to make a real difference to people's lives.

Support the operational requirements of the service by assisting with/taking responsibility for the day-to-day administrative tasks required, such as maintaining rotas, dealing with incoming mail correspondence etc.

We enable vulnerable people at home and overseas to prepare for and respond to emergencies in their own communities. And when the crisis is over, we help people recover and move on with their lives.
