The Carers Breaks Dementia Engagement Team (CBDET) works with Carers  and Vulnerable adults providing person centred support to engage people in community based services, promoting well being and independance. The CBDET team is extending its remit  to offer all older people meeting ASC elegibility criteria the engagement and support service delivered by the team.  

As a Community Support Worker your work will focus on short term engagement with the client and carer to explore opportunities that are personally meaningful to the client and carer and introduce services delivered by the private sector , Voluntary sector and statutory sector providers. You will be required to work in partnership with colleagues in Adult Social Care, the NHS, and the voluntary and private sector to promote personalisation through choice, independence and social inclusion.  The opportunity is there to contribute to creative support packages and make a real difference for carers and clients. The role is demanding and rewarding and requires you to use your own initiative whilst also being a team player. 

Supervision and training will be available.  All secondments require your Manager’s approval.  Candidates need to be car drivers with use of a car or able to demonstrate how the requirements of the post will be met without driving.Additional Information

Hours of work: 37 hours per week Working pattern: Full-time Fixed term for: 15 Months
